In consideration of my athlete being allowed to participate in Gallagher Camps programs, I, the parent or legal guardian named above, acknowledge and agree to the following on my athlete's behalf.
Assumption of risk. Athletic training and competition involve inherent risks of physical injury. I understand these risks and voluntarily accept them as a condition of my athlete's participation.
Release of liability. I release Gallagher Camps and its coaches, staff, and volunteers from any claim for injury, loss, or damage arising out of participation, except where caused by gross negligence or willful misconduct.
Medical authorization. If my athlete is injured and I cannot be reached, I authorize Gallagher Camps to arrange emergency medical care.
Media release. I grant permission for photos and video taken during programs to be used to promote Gallagher Camps, unless I notify Gallagher Camps in writing that I do not consent.
